Remove more vestiges of old specials handling.
* eval.c (struct bindings_helper_vars): Remove de member.
(copy_bh_env_handler): Remove copying of dynamic environment.
(bindings_helper): Remove all code dealing with de.
The sequential binding will no longer introduce a dynamic
environment.
